We are a community owned and operated organisation located on the NSW Mid North Coast.
We have supported elderly residents to live well since 1974. We continue to grow our services, keeping the community and our clients at the forefront of every decision.
Looking for a full time RN to join our Home Care team.
Duties/Responsibilities:
· Conduct comprehensive health assessments and care planning
· Administer medications and monitor for side effects
· Provide wound care and other clinical treatments
· Monitor vital signs and manage chronic conditions
· Coordinate care with GPs, allied health, and family members
· Supervise and support care staff (e.g. personal care workers)
· Educate clients and families on health management
· Maintain accurate clinical documentation and reports
· Respond to changes in client condition and escalate as needed
· Ensure compliance with aged care standards and policies
About you:
· Registered with Nursing and Midwifery Board of NSW (or eligibility) and current Practicing Certificate
· Commonwealth accreditation
· Three years post registration experience preferred, but not essential
· Experience working with older people in a community/home care setting
· Essential requirement to have experience working with older people in a community or home care setting
· Understanding of clinical documentation process
· NSW Full Drivers Licence
· Some understanding of age-related frailties sand illnesses including dementia care
· Aware of difference between medical based model of care and wellbeing-based models of care and the importance of good clinical outcomes for older people
· Valid police check required
