About the Role

We are seeking an experienced and motivated Home and Community Care Coordinator to join our Care team in Coffs Harbour.

This role facilitates support services for members to enable them to live independently at home. It involves development of personalised care plans and budgets and liaison between members, families and health professionals to ensure high quality care

Key Responsibilities

·         Coordination of member services tailored to individual needs, delivered within budget and service expectations

·         Assist with member assessment and help plan development

·         Use a person-centred approach with members, their families and healthcare professionals

·         Oversight of staff, including rostering and maintenance of professional conduct

 

About You

  • Hold (or are working toward completion of) Certificate III in Individual Support, Health Services Assistant or equivalent or Cert IV in Aged Care, Disability or Community Services or equivalent
  • Current NSW Driver’s Licence
  • Awareness of the Aged Care Quality Standards, continuous improvement and home and community care support programs
  • Demonstrated effective interpersonal and communication skills
  • Demonstrated support coordination skills for members with complex health needs
  • Experience working within a multi-disciplinary service model
  • Reliability, punctuality, and integrity

 

Mandatory Requirements

Applicants must hold or be willing to obtain:

·         Current National Police Check

·         Valid Australian work rights

·         Relevant training or willingness to complete training (First Aid, Manual Handling, Infection Control, etc.)
