NVC Group is seeking a Home and Community Care Coordinator to be responsible for overseeing community programs including home care packages, flexible care packages, private home support and brokerage services.
This position is based at our office in Kempsey.

Essential criteria:
- at least one of the following qualifications (or working toward the completion of): Diploma of Nursing/Community Services/Ageing Studies and Services Certificate III Individual Support/Health Services Assistance Certificate IV in Aged Care/Disability/Community Services
- excellent communication skills (both written and verbal) and the ability to build community networks to enhance service delivery to our clients
- sound level of computer skills with the ability to undertake administrative tasks
- demonstrated skills/experience to effectively coordinate for people with complex support needs by identifying and engaging successfully with various service types
- demonstrated ability to manage competing tasks simultaneously, providing excellent outcomes in a cost-efficient manner
- demonstrated ability to be self-motivated and work with limited direction
- commitment to continuing education and professional development
- current driver’s licence.
Desirable:
- experience and knowledge of community services
