Home & Community Care Coordinator:
NVC Group is seeking a Home & Community Care Coordinator responsible for overseeing community programs including home care packages (HCP), commonwealth home support services (CHSP), flexible care packages, private home support and brokerage services.
As a Not for Profit (NFP) organisation, this enables part of your salary to be packaged into pre-tax dollars to pay for personal expenses. Salary sacrifice can be applied to a vehicle, mortgage, rent or EML card.
This immediate start position is based at our head office at Macksville.
Essential criteria:
- Excellent communication skills (both written and verbal) and the ability to build community networks to enhance service delivery to our clients.
- Sound level of computer skills with the ability to undertake administrative tasks.
- Demonstrated skills/experience to effectively coordinate for people with complex support needs by identifying and engaging successfully with various service types.
- Demonstrated ability to manage competing tasks simultaneously, providing excellent outcomes in a cost-efficient manner.
- Demonstrated ability to be self-motivated and work with limited direction.
- Commitment to continuing education and professional development.
- Current drivers’ licence
Desirable:
- Experience and knowledge of community services
- Tertiary qualifications in health care or management preferably with an aged care focus
- Membership in any Industry Reference Groups, Committees or Associations
